A literature search of PubMed and Embase through to January 2007 was conducted to quantify the link between diabetes and endometrial cancer based on a meta-analysis of case-control and cohort studies. Summary relative risks (RRs) with 95% CIs were calculated using random-effects model. Among the 16 studies identified, 3 were cohort studies and 13 case-control studies, including a total of 96003 participants and 7596 cases of endometrial cancer. The meta-analysis revealed that diabetes (mostly of the Type 2 type) was significantly associated with an increased risk of endometrial cancer (RR=2.10), with somewhat stronger risk estimates among case-control studies versus cohort studies, stronger among studies adjusted only for age versus multivariate adjustment, and slightly lower risk estimates among studies performed in the USA versus Europe. Among the three studies focusing on type 1 diabetes, a statistically significant positive association (RR=3.15) was also found.
