Elevated urinary albumin excretion as risk factor for diabetes mellitus in men

This prospective study aimed to assess the association between baseline urinary albumin excretion (UAE) and subsequent diabetes mellitus incidence in 4074 nondiabetic patients participating in the French epidemiological study DESIR on the insulin resistance syndrome. During the 9-year follow-up, diabetes mellitus occurred in 171 out of 3851 patients with complete data regarding diabetes mellitus diagnosis. Baseline UAE was associated with incident diabetes mellitus in a dose-dependent manner in men, but not in women, the association being more marked after excluding men with impaired fasting glucose at baseline. After adjusting for confounding variables, this association remained significant. Further adjustment on the first 3-year change in weight, glucose, insulin and insulin resistance (HOMA-IR) did not modify these results. In conclusion, independent of baseline and of early development of metabolic abnormalities or insulin resistance, elevated UAE predicts 9-year risk of diabetes mellitus in men.
